Dr. Sabir explained that NCCLs often appear as smooth saucer-shaped defects or V-shaped notches, are more common in older patients, and are frequently found on the facial surfaces of maxillary premolars. Major contributing factors include abrasion from aggressive brushing, erosion from acidic foods and drinks, and possibly occlusal stress or abfraction, though the latter remains controversial.<br /><br />Diagnosis relies on careful medical and dental history, identification of risk factors, and clinical assessment of lesion shape, depth, sensitivity, and location. Management is centered first on prevention: reducing risk factors, improving oral hygiene habits, using fluoride and potassium nitrate products, and monitoring progression regularly. Restorative treatment is delayed when possible but becomes appropriate when lesions are large, progressive, symptomatic, or compromise aesthetics or function.<br /><br />For restorations, composite resin—especially flowable composite—is recommended. Proper isolation, conservative preparation, enamel beveling, selective etching, and careful bonding are emphasized. Large lesions may benefit from a flowable base with a more wear-resistant composite overlay. Ongoing follow-up every 3 to 6 months is important for both preventive and restorative cases.
