He also discussed pain patterns after root canal therapy and periodontal grafting, showing that pain duration and intensity vary by procedure and tissue thickness.<br /><br />Key evidence from Cochrane reviews showed that ibuprofen plus acetaminophen has one of the best analgesic outcomes, with an NNT around 1.6, outperforming many opioid regimens. Diclofenac and ketoprofen were noted as effective “lone ranger” NSAIDs, while codeine and hydrocodone combinations performed poorly. NSAIDs were described as having important anti-inflammatory benefits but also risks such as gastrointestinal irritation, renal effects, and prolonged bleeding time.<br /><br />The speaker recommended provider-directed analgesia, especially ibuprofen 400 mg plus acetaminophen 1000 mg every 8 hours, as first-line therapy for moderate to severe dental pain when not contraindicated. Corticosteroids were shown to work best when given preoperatively and by parenteral route.<br /><br />The presentation ended with newer agents, including suzetrigine (Journavx), a sodium-channel blocker, and other emerging pain therapies. Overall, the message was to prioritize evidence-based non-opioid treatment and reserve opioids for specific contraindications or special cases.
