The webinar introduced Lieutenant Colonel Preston S. Duffin’s presentation on TMD diagnosis and treatment, aimed at helping dentists better understand and manage temporomandibular disorders. He emphasized that TMD is common, especially in young adults and middle-aged patients, and occurs more often in females. Risk factors are multifactorial and include stress, anxiety, depression, poor sleep, parafunctional habits like clenching, trauma, hypermobility, genetic susceptibility, and other chronic pain conditions.<br /><br />Duffin reviewed major classification systems used in oral and facial pain, especially ICOP and the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). He explained that DC/TMD helps categorize common joint, muscle, and headache-related disorders, including arthralgia, arthritis, disc displacement with or without reduction, hypomobility and hypermobility disorders, osteoarthritis, condylar resorption, myalgia, myofascial pain, tendonitis, spasm, and headache attributed to TMD.<br /><br />A major theme was that history is often more important than imaging in diagnosis. Familiar pain, joint noises, movement limitations, and reproduction of symptoms during exam are key. He stressed the importance of distinguishing acute from chronic pain, noting that chronic TMD often involves central sensitization and requires attention to psychosocial and behavioral contributors.<br /><br />Treatment should begin conservatively: patient education, reassurance, habit modification, jaw rest, stretching, physical therapy, and behavioral approaches such as CBT and biofeedback. Medications may include NSAIDs, steroids, muscle relaxants, TCAs, anticonvulsants, or topicals depending on the pain type. Appliances, trigger point injections, acupuncture, and other interventions may help, but surgery is reserved for severe cases. The lecture concluded with recommended resources for further study.
