Video Summary
Dr. Thomas M. Pommier presented a webinar on the evidence linking oral health and systemic disease, focusing on how dentists and hygienists should interpret research and apply it clinically. He reviewed evidence-based dentistry concepts, explaining the differences between observational studies, cohort studies, case-control studies, randomized controlled trials, and meta-analyses, while stressing the importance of confounders, confidence intervals, and study quality. He also warned that research can be misleading through “spin” and that clinicians must read beyond abstracts and headlines.

The lecture traced the history of the old “focal infection theory,” noting that modern science does support real links between oral bacteria, inflammation, and distant disease, but not the extreme idea that all teeth should be extracted. Dr. Pommier emphasized that periodontal disease is a chronic inflammatory condition involving biofilm, cytokines, and immune dysfunction, and that it is associated with many systemic conditions, though true causality is proven mainly for diabetes.

He reviewed evidence connecting oral disease with diabetes, metabolic syndrome, obesity, COVID-19, cardiovascular disease, and mortality. Examples included higher risk of apical periodontitis in diabetics, worse COVID outcomes in patients with periodontitis, and associations between periodontal disease and heart attack, heart failure, and all-cause mortality. He highlighted biologic plausibility through bacteria, inflammation, and immune responses, and encouraged stronger home care, professional maintenance, and patient education. The overarching message was that oral health is inseparable from overall health.
